​​Dementia is a difficult disease to handle, whether you are a patient or a caregiver. This is why Medicare has started the GUIDE program with no copays to help dementia patients and their caregivers. In addition, in recognition of the value caregivers provide in patient care, Medicare will cover $2500 of respite care annually to support the caregiver.

 

Embrace provides these services in areas in Delaware and South-East Pennsylvania. Coverage is limited to specific zip codes. Check with us today to see whether you are eligible for this new program.

(GUIDE) Model Goals:  ​

GUIDE Care Delivery Services Include

Medication Management and Reconciliation

Ensures the right medications are taken at the right times.

Ongoing Monitoring and Support

Care Navigators provide guidance via frequent telehealth visits in the comfort of your home.

Caregiver Respite Services

Caregivers are supported with temporary respite services of $2,500 per year paid by Medicare.

Care Team Coordination

Care team coordination improves patient care and reduces caregiver burden.

Medication Management and Reconciliation

Ensures the right medications are taken at the right times.

All GUIDE services are provided with no cost or copayments for qualifying Medicare beneficiaries.

Eligibility for participation in GUIDE is limited to beneficiaries with diagnosed dementia and who have Traditional Medicare as their primary insurance. At this time, GUIDE is not available for patients with Medicare Advantage, those residing in a skilled nursing facility, or patients enrolled in PACE or hospice programs.

GUIDE allows us to have a dedicated Care Navigator work regularly with you and your caregiver virtually to address your specific needs, answer your questions and coordinate your care to help keep you safely in your home.
